How to Apply for Access Service

Before beginning the referral process, call our toll-free number, 1-844-373-4515. This will give you the opportunity to ask questions about our services and discuss your situation.

  • The application process starts with your requesting the service directly or by a referral, a parent’s lawyer, a judge or the Office of the Children’s Lawyer (OCL).
  • Referrals should include a copy of the court order or the signed letter of agreement and should be faxed to our Program Administrator at 905-953-8241.
  • The court order or endorsement or the letter of agreement (signed by both parents) must specify the conditions of access, which include the number of hours per visit, the number visits per week, restrictions, and the names of the individuals who can attend the visits.
  • We also need, in relation to custody and access, any social work assessments and/or psychiatric reports involving either parent, a copy of the bail order, probation order and/or restraining order, if applicable.
  • The written referral should include full names, addresses, telephone numbers for both parents and lawyers, and the full name and date of birth of the child(ren) involved.    
  • All referrals are accepted on a first-come, first-served basis. If space is not available, you will be placed on a waiting list and contacted as soon as space becomes available.
